Elementary School, Grades PreK-6

If you are new to the district and need to register your grade K-6 child, please utilize the editable documents below and email them to [email protected]. Health assessments may be faxed to 716-337-0598. If copies need to be made of any documents (parent/guardian IDs, birth certificates, proof of residency, etc.), please make an appointment with Melanie Schroeder by phone at 716-337-0166, ext. 2151, or by email at [email protected]. In addition, we will need the following documents to register your child:

  1. Current report card
  2. Record of immunization
  3. Birth or Baptismal Certificate 
  4. Completed Registration Packet (click link at the bottom of this page to download)

Kindergarten Registration for 2024-2025

If you have a child who will be 5 years old on or before December 1st, 2024 and does NOT attend Pre-K at North Collins Elementary, please complete the registration paperwork below.

Pre-K Registration for 2024-2025

If you have a child who will be 4 years old on or before December 1st, 2024, please complete a PK Pre-Registration Application by Thursday, March 28, 2024.  In addition, a full registration packet must be completed by Friday, May 31, 2024. If there are more than 36 fully completed applications for prekindergarten students by the end of the day on 3/28/24, there will be a lottery held in April for those spots.

Jr./Sr. High School, Grades 7-12

Below are the forms necessary for a parent/guardian to fill out in order to enroll a NEW student in North Collins Jr.-Sr. High School, as well as additional information about our school.

Notice of Intention to Instruct at Home

The parents or other persons in parental relation to students of compulsory education age wishing to educate their children at home, must notify the Superintendent of Schools in writing each year by  July 1 of their intention to educate their children at home. Letters of intent can be sent to:
